Change of Address Form

Please complete this form and return to your campus Registry Office. Alternatively you can update your address details on your student portal.

Change of Name Form

Please complete this form and return to your campus Registry Office, you will also need legal documentation if you wish to change your name.

Course Transfer Form

This form should be completed in consultation with your Course Director.

EC1 Form

 

Exemption Form

This form should be completed if you wish to apply for module exemptions on the basis of previous study.

Leave of Absence Form

Please ensure you have read the Deregistration Policy and consulted with your Course Director before completing a leave of absence form.

Module Amendment Form

This form should be completed in consultation with your Course Director. Module changes should take place within the first two weeks of term.

Request for Refund of Fees Paid to Examinations Office Form

 

Replacement Student Card Form

This form should be completed to request a replacement student ID card or alternatively you can call into your campus Registry Office.

Request for posting of Parchment Form

 

SA1 Form

Appeal on the basis of new information not previously available to the examiners.

SA2 Form

Appeal on the basis of procedural and other irregularities.
